Software deployments is based on specific set of instructions that need to be accurately followed in order to have a working software in production. Based on the observations made in several organisations, deployments are still done manually, which increases the the risk of human error, lack of consistency and repeatability. 

Also, deployment instructions are often poorly defined, documented and revised, which increases rate of post-deployment rollback and incidence of high severity incidents in the initial hours the new system went live.

Finally, the lack of adoption of a strong process is prejuducial even in smaller organisations, where the costs of failed deployments have a higher financial impact and should be avoided.
